You may choose to use a fill-in-the-blank template that names a person or situation and makes a request of God.

   For my church that through our fellowship the lonely would find a home and a family.

   For my friend that their anxiety would be eased.

A journal can be a good place to keep a list of such intercessions. You may also record how the Lord is present in the needs of those for whom you intercede.
